Press Ctrl + J to duplicate the layer.

In the menu click Filter, Blur and choose Radial Blur...
In the new settings window, set the amount to about 88 and Zoom for a Blur Method.
You can click and move the Blue Center to set the center point of the effect where on your photo you want it to be.
I'm going to move it towards the head.
Click OK.

In the menu click Image, Adjustments and choose Desaturate (Shift + Ctrl + U for a keyboard shortcut).

Set the blending mode of the duplicated layer to Hard Light.
|
|
|

It should look like similar to mine at this point.

Now again, press Ctrl + J to duplicate the duplicated layer and set the blending mode of the new layer to Screen (see picture).
