Let's make it look, like someone's face part is shot with the iPhone's camer and you can see that part on the phone's screen.

1.

Open a picture with a smiling face.

I've chosen the pretty smile of Jessica Alba.

2.

Click File and Place. And browse for the  picture of iPhone or any other Phone.

Just be sure it's a picture of the phone's display side.

3.

And press Enter.

On my picture you can see the iPhone from two perspectives. We only need the one with the display, that's why we're going to extract it from the picture.

4.

Click the Quick Selection Tool.

5.

And select the whole phone with the display.

6.

Click Select and Refine Edge. (Alt+Ctrl+R for keyboard shortcut)

7.

Adjust Smooth and Feather a bit.

I've set Smooth to 7 and Feather to 1.4.

8.

Click Select and Inverse (Shift+Ctrl+I for keyboard shortcut).

9.

Right click the phone layer and click Rasterize Layer.

Press the Delete key to delete everything but the Phone you need.

10.

Click Select and Deselect (Ctrl+D for keyboard shortcut)

11.

Press Ctrl+T then right click the phone and click Rotate 90 CCV

12.

Drag the phone so the display covers the mouth.

13.

Click Rectangular Marquee Tool ...

14.

...and select the phone's display.

15.

Click the Background layer.

16.

Click Layer, New and Layer via Copy (Ctrl+J for keyboard shortcut)

17.

A new layer - Layer 1 should appear, click and drag it to the top.

18.

This is how it should look like.

19.

Right click the Background layer and click Layer From Background.

20.

Press Ctrl+T then click the lower right corner of your face picture, press the shift key and hold it then drag your mouse a bit to the middle of the picture to shrink it a bit.

Press Enter.

21.

Click and drag the face picture to adjust it with the smile on the phone.

22.

Click Crop Tool.

23.

And crop the picture.

It's just like you would select it with Rectangular Select Marquee.

Press Enter to finish the crop.

24.

This is how my end result looks like :)
